The lateral occipitotemporal gyrus, occupying the ventral surface of the temporal lobe between the inferior temporal gyrus laterally and the parahippocampal gyrus medially. Bounded below by the collateral sulcus and above by the occipitotemporal sulcus. Continues anteriorly with the temporal pole and posteriorly with the lateral occipital cortex.

The fusiform gyrus is the cortical seat of object- and category-specific visual processing. Its central segment houses the fusiform face area (FFA, around MNI -40, -55, -20), a region that responds preferentially to faces over other object categories in essentially every neurotypical participant tested. Adjacent territories include the visual word form area (VWFA, more anterior and lateral on the left fusiform), the lateral occipital complex (for general object recognition), and category-selective patches for bodies, places, and tools.

Disruption of right fusiform cortex selectively impairs face recognition (prosopagnosia) while preserving general object recognition. Left fusiform damage classically produces pure alexia, with intact spelling and recognition of letters traced on the skin but inability to read printed text. The dissociation between face and word recognition in the fusiform underwrites a long-standing debate about whether categories are encoded in dedicated cortical patches or in distributed activity across the ventral visual stream.

The fusiform face area was identified by Kanwisher, McDermott, and Chun in 1997 using functional MRI contrasts comparing faces with other object categories. The reliability of the FFA across subjects, and the selectivity of its response to faces, made it the canonical example of domain-specific cortical organisation and the subject of two decades of subsequent argument about whether such category-specific patches reflect innate organisation, expert-level learning, or representational clustering within a continuous ventral-stream map.

The visual word form area lies anterior and lateral to FFA on the left mid-fusiform gyrus. Its anatomical reproducibility across literate individuals, despite the cultural and evolutionary recency of written language, is one of the cleanest demonstrations that cortical specialisations can arise through experience-dependent recycling of pre-existing object-recognition circuitry (Dehaene’s “neuronal recycling” hypothesis).

Both face and word recognition are strongly lateralised but in opposite directions: face processing is right-dominant, word processing is left-dominant. This complementarity may reflect competition for cortical real estate during literacy acquisition, with reading recruiting left-fusiform territory that in non-literate individuals contributes more to face recognition.
